CBT Thought Record Template (Printable PDF)

A CBT thought record is a one-page worksheet that walks one upsetting moment through seven columns: the situation, the emotion rated 0–100, the automatic thought, the evidence for it, the evidence against it, a balanced thought, and a re-rating of the emotion. Print the blank template below or download the PDF — no email needed — and fill in one record the next time a feeling spikes and won’t settle.

How to use it

  1. Fill it in while it’s warm. Use the sheet within an hour or so of the feeling spiking, not at the end of the week. The exact wording of the thought fades fast, and the exact wording is what you’re testing.

  2. Facts first, then the feeling. Situation is only what a camera would have recorded. Then name each emotion and give it a number from 0 to 100 — that number is the before-photo the last column measures against.

  3. Write the thought word for word, then give it a fair trial. Copy the thought as it arrived, melodrama included. Steelman it in “evidence for”; then list facts — not reassurance — in “evidence against”.

  4. Write a balanced thought and re-rate. One sentence that accounts for both evidence columns, then the same emotions with new numbers. A drop of a few dozen points, not zero, is a good result.

CBT Thought Record

Date

One upsetting moment, seven columns. Rate emotions 0–100. Write the thought exactly as it sounded.

1Situationwhen, where, who — facts a camera would catch
Tuesday, 6:40pm. Message from my manager: “Can we talk tomorrow?”
2Emotionsname each, rate 0–100
Anxious 85 · dread 70
3Automatic thoughtword for word · belief 0–100%
“I’m getting fired.” (belief 80%)
4Evidence forfacts that support it — steelman it
The message has no context. There were budget-cut rumors last month.
5Evidence againstfacts, not reassurance
My last review was strong. She booked a normal 1:1, not an HR meeting. She asked me to lead the Q3 project two weeks ago. Every previous “can we talk” was logistics.
6Balanced thoughtone sentence that honors both evidence columns
A context-free message is ambiguous, and the track record says it’s most likely routine. I’ll know tomorrow; tonight I only have a blank, not a verdict.
7Re-ratethe same emotions, new numbers
Anxious 85 → 55

Common questions

What are the seven columns of a thought record?

Situation (facts only), emotion with a 0–100 rating, automatic thought (word for word), evidence for the thought, evidence against it, a balanced thought that accounts for both, and a re-rating of the same emotion. The template prints all seven on one page.

How often should I fill in a thought record?

Whenever a feeling spikes and won’t settle — one record per moment. A single record usually brings some relief; the questioning starts to happen on its own only after repeated practice, so reprint the page and keep going for a few weeks.

Do I need to give an email to download it?

No. Print it from this page or download the PDF directly — no email, no sign-up. If you want more, the 16-page Reframe Workbook is a separate, optional download that is sent with a newsletter opt-in.

Will it print on A4 and US Letter?

Yes. The sheet is sized to fit inside both A4 and US Letter margins, so it prints on one page at 100% scale. The Print button prints only the blank sheet — the navigation, explanations and buttons are hidden.

Can I use this without a therapist?

Yes, as a self-help skills tool. It uses the same format CBT therapists use, but it is not treatment and does not replace it. If you are in crisis or thinking about harming yourself, contact local emergency services or a crisis line (in the US, call or text 988).
